To obtain admission information to the Distance Learning Endorsement program you have a choice of formats:
If you have Microsoft Word on your computer click here.
If you do not have Microsoft Word click here.

You may submit the form by either of the following methods:
1. Fax the above to David Stokes at 487-9507 if you are sending from the Salt Lake City area or 801-487-9507 from outside of Salt Lake City.
2. Save this form to your hard drive, complete the data and email as an attachment to dstokes@westminstercollege.edu

3. Your information will be sent on to our admissions office for the formal application process to begin.
